Republic County Hospital JOB TITLE: Activity Director Full-time M-F; some holidays and weekends may be required DEPARTMENT: Intermediate Swing Bed

JOB RELATIONSHIPS:

Reports: Responsible To: Registered Nurse, Licensed Practical Nurse, Swing Bed Coordinator, and ISB Department Manager. Responsible For: Does not supervise other employees. Interrelationships: Works cooperatively with clinic and hospital departments, medical staff, employees, residents, and visitors. JOB SUMMARY: The Activities Director is a professional caregiver who provides care, activities, and restorative exercises under the direct supervision of licensed staff.

JOB QUALIFICATIONS:

Experience: Previous experience in a health care setting preferred, not required. Education: High School graduate (or equivalent). Must be 18 years or older. Current CNA Kansas Certificate required. CPR certificate must be obtained and maintained upon employment. Activities Director Certification preferred, not required. Must be obtained and maintained within a year of employment in position. Must have a valid Driver’s License “In Good Standing” and must complete vehicle checklist with hospital staff.

KEY COMPETENCIES:

Excellent written and verbal communication. Knowledge in MS Office, Gmail, and patient management software. Strong organizational, planning, time management and prioritizing skills. Ability to multitask. Critical thinking. Able to follow policy and procedures. This list is not exhaustive and may be supplement as necessary.

JOB RESPONSIBILITIES:

Prepare unit/patient room for receiving a patient/resident for admission. Provide personal care assistance including: bathing, hygiene, grooming, dressing, eating, toileting, transferring, and ambulating. Obtain and document vital signs, weight and intake/output as directed. Document accurately daily activities and pertinent information. Make regular rounds to assigned patients, anticipate their needs, and answer and respond to call lights in a timely manner. Transport patient/resident as necessary, including utilization of transport vehicle. Provide postmortem care when necessary. Work with other team members to turn and transfer patients as directed. Plan, prepare, coordinate, and offer activities for all skilled and intermediate swing bed patients. Document activities offered, participation, and alternate activities offered as appropriate in EHR. Will complete activity assessment within 72 hours upon admission and with changes of level of care with all skilled and intermediate swing bed patients. Skilled patient will need a weekly review of assessment; Intermediate Swing Bed patients will have an updated activity assessment with resident’s periodic comprehensive review and as needed. Participate in patient’s care plan meeting. Plan and post an activity calendar. Encourage patients to attend activities. If patient refuses majority of group activities, the director will plan and help implement one-on-one activities to meet patient need and choices. Coordinates operation of Resident and Food Council meetings. Participates in Quality Improvement projects as directed. Implement restorative care plan in conjunction with the Rehab department and will inform Rehab staff of any noted changes in physical or mental abilities. WORKING CONDITIONS:

Occasionally exposed to the risk of blood borne diseases. Contact with patient/residents under a wide variety of circumstances. Occasionally exposed to unpleasant elements (accidents, injuries, and illness). Subject to varying and unpredictable situations.

ESSENTIAL FUNCTIONS:

Stand, walk, bend, stoop and stretch for prolonged periods of time. Push/pull patients in wheelchairs (up to approximately 250 lbs.) Restocking supplies. Working with equipment.
